Records held
- Patient clinical records and correspondence.
- Accounting, billing and medical-scheme records.
- Statutory records required by the Companies Act, the Income Tax Act and the HPCSA.
- Employment and service-provider records.
How to request a record
Requests must be made on the prescribed PAIA form and delivered to the Information Officer at the email address above. The request must identify the record sought, the right you seek to exercise, and the form of access required. A response will be provided within 30 days, extendable in the circumstances allowed by PAIA.
Grounds for refusal
Access may be refused where PAIA requires or permits it, including to protect the privacy of a third party, medical records where disclosure would be harmful, legally privileged material, and confidential commercial information.
Your POPIA rights
- Access to the personal information we hold about you.
- Correction or deletion of inaccurate, irrelevant or outdated information.
- Objection to processing on reasonable grounds.
- Complaint to the Information Regulator at complaints.IR@justice.gov.za.
